About me

I’m a Registered Nurse with over 16 years of experience caring for patients at Henry Ford Hospital and Corewell Health. My heart for helping seniors began when I was a teen, helping care for my great-grandmother after her debilitating stroke. That experience shaped my passion for ensuring older adults feel safe, valued, and supported.

As the founder of BrightGuide, my mission is to simplify the complexities of care and provide expert support with evolving medical needs, hospital transitions, and practical help with the everyday challenges of aging. My goal is to be a care partner that families can trust and genuinely rely on for help when they need it most.

At BrightGuide, our mission is to enhance the health & wellbeing of seniors with personalized support through nurse-led care coordination.

We empower seniors to live at home with confidence. With consistent oversight from nurse visits and care-partner support, BrightGuide helps seniors maintain independence while giving families reassurance knowing their loved one is being continually monitored.

Through clear communication and collaborative advocacy, we help bridge the gaps between families, caregivers, and healthcare providers — ensuring peace of mind and a more fulfilling aging experience for everyone involved.

Services

Initial Wellness Consultation

Your visit begins with a comprehensive in-home assessment of physical, emotional, medical, and safety needs. This includes a fall-risk and home safety evaluation, medication review, and gathering relevant medical information.

Using this, a personalized care plan is created outlining recommendations to address current concerns, limitations, and future goals. A service package is created to meet your needs, and the licensed BrightGuide nurse is ready to become your dedicated care partner.

In-Home Wellness Visits & Health Assessment

Routine nurse visits combining health assessments, vital signs and symptom monitoring, fall-risk and safety evaluations, medication assistance, and personal companionship for added support and social interaction.

Emergency & Hospital Support

On-call nurse support during hospitalizations and transitions of care. May include hospital or emergency room accompaniment, post-hospital follow-up visits, reinforcement of discharge instructions, and monitoring for changes in health after returning home.

Appointment Assistance & Transportation

Attending and coordinating medical visits. May include transportation to and from appointments, advocacy and communication with physicians, clarifying treatment plans, and sharing post-visit summaries with families.

Medication Assistance

Hands-on nursing support to ensure safe and accurate medication use, including medication review, education, monitoring for adverse side effects, and coordination with physicians & pharmacies as needed.

Home Care Planning

Assistance with selecting, organizing, and overseeing in-home care services including home health aides, therapy, or other specialized care.

Family & Caregiver Advocacy

Support for caregivers and families in managing responsibilities, making care decisions, and planning for long-term needs. Optional in-person or virtual family meetings can be organized and facilitated as needed.

Safety & Technology

Integrating smart devices and various technologies to support independence, reduces risk of falls and injuries, and keeps caregivers connected.

Household Care Management

Coordination of several home services including housekeeping, lawn care, food services, personal care, and transportation.
